A science-backed breakdown of the Paleolithic diet - what it includes, who it suits, and the real evidence behind its weight loss and metabolic claims.

The Paleo diet, also known as the Caveman or Stone-Age diet, has become one of the most talked-about eating patterns for people chasing weight loss, better blood sugar control, and a leaner physique. Modern Western diets - high in calories, fat, glycemic load, processed meat, and added fructose - are closely linked to the rise of chronic diseases such as obesity, cardiovascular disease, type 2 diabetes, and non-alcoholic fatty liver disease.

The Paleo diet argues that our genes are still adapted to the wild, hunted, gathered, and fished foods of our ancestors rather than the farmed, processed staples of today. So what exactly does the Paleo diet involve, who is it suitable for, and what does the science actually say about its pros and cons?

What exactly is the Paleo diet?
🦴  A throwback to Stone-Age eating

The Paleo diet mimics the dietary practices of the Palaeolithic or Stone-Age era, spanning roughly 2.5 million to 10,000 years B.C. During this period, humans relied on wild food sources - mainly meat, insects, green vegetables, roots, organ meats, seasonal fruit, and nuts. The Paleo diet became popular as people sought to improve body composition by avoiding heavily processed foods, and it is now widely practiced among athletes and young adults.

Nutritionally, the Paleo diet is characterised by being low in carbohydrates (around 24.5% of intake), high in protein (around 29.6%), and rich in polyunsaturated fatty acids (around 15.3%) and cholesterol (roughly 835mg per day). Because Stone-Age humans could not milk wild animals or easily process cereal grains, dairy and grains are excluded. Salt was not used, and the only refined sugar available to our ancestors was honey.

The Paleo diet favours whole, unprocessed foods that could theoretically be hunted, fished, or foraged, while excluding farmed staples such as grains, legumes, and dairy. Here is the full breakdown:

✅  Promising, but evidence is still limited

Several clinical studies have examined the Paleo diet's effect on metabolic health. Lindeberg et al. (2007) compared a Mediterranean-like diet with a Paleo diet in individuals with ischaemic heart disease. The Paleo diet group showed significantly greater improvement in glucose tolerance - a key marker used to assess diabetes risk and insulin resistance - along with significantly more weight loss. Waist circumference reduction was also notably higher in the Paleo group (5.6cm) compared to the Mediterranean group (2.9cm).

A separate randomised controlled pilot study by Boers et al. (2014) found that while the Paleo diet did not significantly impact glucose tolerance, it did help lower plasma lipid levels and blood pressure, alongside a decrease in body weight.

Together, these findings suggest the Paleo diet can meaningfully improve metabolic parameters. However, due to short study durations, small sample sizes, and limited controls, there is not yet strong enough evidence to confirm long-term benefits for type 2 diabetes or other chronic diseases. That said, the Paleo diet's emphasis on natural fruits, vegetables, oils, nuts, and seeds - while avoiding refined sugars and processed, energy-dense foods - does offer plausible anti-inflammatory benefits.

⚠️  Adjustment symptoms are common in the first few weeks

Because the Paleo diet sharply cuts carbohydrates and removes dairy products entirely, the transition from a high-carb to a low-carb intake can be challenging at first. Common side effects may resolve within a few days, though in more serious cases they can last two to four weeks. These include:

  • Headache
  • Fatigue and lack of energy
  • Nausea and dizziness
  • Body aches and cold sweats
  • Constipation and/or diarrhea

Beyond the initial adjustment period, eliminating dairy products on the Paleo diet may lead to inadequate calcium intake, which is important for bone and teeth development. The diet can also be difficult for vegetarians to follow since it excludes beans, a key plant protein source. Additionally, because the Paleo diet restricts grains - and the insoluble fibre they provide - its long-term impact on gut health remains unclear.

The Paleo diet is generally most suitable for healthy adults, including athletes and young adults, who want to lose weight, support diabetes management, or improve general metabolic markers such as blood pressure and lipid levels. It may be less suitable for vegetarians and vegans, given the heavy reliance on animal protein and exclusion of legumes, and individuals with existing calcium deficiencies or bone health concerns should pay close attention to alternative calcium sources once dairy is removed.
